By receiving telehealth services through Drip Vitals, you acknowledge that you have read and understand this consent and agree to receive healthcare services through telemedicine.

For sensitive medical information, patients are encouraged to use the secure Drip Vitals patient portal whenever available.

The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed in the same manner as FDA-approved medications.
Compounded medications are not automatically equivalent or interchangeable with FDA-approved branded or generic products.
You should follow the prescription label and instructions provided by your dispensing pharmacy.

You may choose not to receive treatment through telehealth.
You may withdraw your consent to receive future telehealth services by notifying Drip Vitals.
Withdrawal of consent will not invalidate medical services already provided.
If you withdraw consent, Drip Vitals may be unable to continue providing healthcare services remotely.
Information generated during your telehealth care may become part of your medical record.

Medical records are maintained in accordance with applicable federal and state privacy, confidentiality, and medical-record laws.
For patients physically located in California, Drip Vitals obtains consent to the use of telehealth before the initial delivery of telehealth services and documents that consent in the patient’s medical record.
California Business and Professions Code § 2290.5 permits telehealth consent to be verbal or written and requires that the consent be documented.
Your electronic acceptance of this Telehealth Informed Consent constitutes consent to receiving telehealth services through Drip Vitals.
For patients physically located in Texas, informed consent must be obtained before telemedicine medical services are provided.
Texas law also requires the confidentiality of patient clinical information to be maintained in connection with telemedicine services.
By electronically accepting this Telehealth Informed Consent before receiving medical care, you consent to receiving telemedicine medical services from Drip Vitals.
Additional telehealth requirements may apply depending on the state where you are physically located.
Drip Vitals may provide additional notices, disclosures, acknowledgments, or consents during registration, medical intake, or before treatment when required by applicable law.
Any additional state-specific consent presented to you becomes part of your telehealth consent.
Before receiving telehealth medical services through Drip Vitals, you will be required to affirmatively accept this Telehealth Informed Consent electronically.
By checking the applicable consent box, electronically signing, or otherwise affirmatively accepting this Telehealth Informed Consent, you acknowledge and agree that:
During the patient intake process, patients will be presented with the following acknowledgment:
“I have read and agree to the Drip Vitals Telehealth Informed Consent. I consent to receiving healthcare services through telehealth and acknowledge that my electronic acceptance will be documented in my patient record.”
Patients must affirmatively accept the consent before proceeding with telehealth medical services.
